To qualify for a $50 Mail- In Rebate:

  1. Purchase a Palm smartphone from http://activate.palm.com with a new two-year wireless service Agreement between June 5, 2008 and July 6, 2008.
  2. Submit a fully completed mail-in rebate redemption form with a photocopy of your original in-box shipment receipt dated between June 5, 2008 and July 6, 2008.
  3. All requests must be postmarked by August 6, 2008 and received by July 31, 2008. Limit of one rebate per household/address, except in Rhode Island where there is a limit of two rebates per household/address.
  4. Offer valid in the United States ONLY.
  5. Void where prohibited by law.
